Getting Ready for your Appointment:

Use this checklist as a convenient resource to make sure you are ready for a prompt check-in and check-out.

  • Caffeine — at least 4 hours before your appointment

    Nicotine — at least 4 hours before your appointment

    Uncomfortable Shoes — wear comfortable, preferably no open-toed shoes, slippers, slides, or sandals

  • Glasses/ Contacts — bring your glasses including distance or near sight, or wear your contacts on appointment day

    Hearing Aids — make sure your batteries are fresh and in working condition

    Inhaler — if you have asthma, or a respiratory condition that may require an inhaler, bring it with you

    Paperwork — if your company provided you with paperwork in advance, fill out your portion before coming to your appointment. If you have questions you can reach out to us or ask on the day of your appointment in the office.

Preparing for DOT Physicals:

  • Stent or prior MI (heart attack) — please request your most recent ECHO or stress test from your cardiologist

    Pacemaker —please request your most recent pacemaker reading from your cardiologist

    Open heart surgery or coronary artery bypass graft (CABG) — request your most recent ECHO or stress test from your cardiologist

  • MAKE SURE TO TAKE YOUR BLOOD PRESSURE MEDICATION AT LEAST 2 HOURS PRIOR TO YOUR APPOINTMENT

  • Request your most recent hemoglobin A1C from your doctor (PCP or endocrinologist)

    MAKE SURE TO TAKE YOUR MEDICATION PRIOR TO YOUR APPOINTMENT (IF APPLICABLE)

    If you use insulin — scroll to the bottom of this page and find the document titled ‘INSULIN WAIVER’, have your doctor, fill it out completely and fax to POHC or bring it in at the time of your appointment.

  • If you are on medication for a mood disorder— scroll to the bottom of this page and find the document titled ‘MOOD DISORDER MEDICATION CLEARANCE’, have your prescribing doctor fill it out completely and fax to POHC, or bring it in at the time of your appointment.

    If you discuss a present mood disorder with your doctor but are not taking medication for it— scroll to the bottom of this page and find the document titled ‘MOOD DISORDER NO MED CLEARANCE’, have your doctor fill it out completely and fax to POHC, or bring it in at the time of your appointment.

  • CPAP machine use—send us or bring with you, your last 90 day report, we will not accept a 30 or 60 day report as they are not sufficient.

    *If you've had your CPAP machine for less than 60 days then we will accept the 30 day report.
